Dermal Fat Grafting


A graft is tissue separated from it own blood supply and transferred to another region.
The blood supply of the new location grows into the graft and nourishes it. Injected fat and dermal-fat grafts are both grafts. Penile girth augmentation was typically performed by fat injection. However, this technique has fallen into disfavor because fat reabsorption and cyst or nodule formation often produce penile asymmetry or other deformities, especially if large fat volumes are injected. Small volumes of fat are not likely to produce as high an incidence of nodules and cysts. However, since less than 50% of the injected fat eventually survives, permanent girth gain is relatively low if a small volume of injected fat is used.

A dermal-fat graft is a solid piece of skin (with the superficial layer removed) with its underlying fat. A dermal graft is a graft of only the skin without underlying fat. Dermal-fat grafts have a greater chance of survival than injected fat, because the injected fat is more traumatized during the transfer and is not as securely placed. Girth can be enhanced by longitudinal insertion of dermal-fat graft strips into the loose tissue located between the erectile bodies and the skin. However, displacement of the grafts and irregular ridges may occur.

Increased Penis Thickness


The preferred augmentation technique consists of wrapping a dermal-fat graft sheet around 3/4 of the circumference of the penis, leaving the urethra (urinary channel) uncovered. The graft is placed directly on the firm elastic tissue covering the erectile bodies after the penile skin is pulled back, usually via a circumcision incision. The skin is then replaced over the graft, which has an excellent chance of surviving. The dermal-fat graft is obtained either from the buttock creases or the inside of the buttock cheeks.

The dermal-fat grafts are accurately placed to create a normal, thicker penis. Results are much better with the wrap than with dermal-fat grafts strips or with fat injections, since the feel and appearance are more natural. Circumference increases 3/4 to two inches, usually permanently.

The girth is not increased so much as to be disproportionate. Once the dermal-fat grafts are placed, the penis will no longer shrink to its small flaccid size, thus solving the problem of the small flaccid penis.
